Boy Scouts Third-Party Witness Statements

We need this information to establish that:

    The person who asked you to sign the forms was a Boy Scout,
    OR
  • You have information that the Third-Party Scoutmaster, Volunteer or someone else was associated with scouting.

PINK FORM

Signed by you (a mom, dad, sibling, friend, fellow Scout, etc.) regarding knowledge that the person who asked you sign was a Boy Scout.

OLIVE FORM

Signed by you (a mom, dad, sibling, friend, fellow Scout, etc.) who has knowledge that a third-party (scoutmaster, volunteer, etc.) was involved in scouting.
